Case Management at SOME is the Foundation for Service Access and Delivery
Introduction
Case Management is the very foundation of service provision at SOME. It is frequently the initial point of contact for clients, and the engagement and relationship building that is done is the root from which individual change begins.
Case management at SOME covers a full range including acquisition of vital records, referral, and linkages in social services, to resident supportive services, problem solving, and supports in housing, transitional service plans and clinical case management for health care services and transitional housing programs.
Case Management at SOME is overseen through the Case Management Practice Group. This group is made up of an appointed social worker and all case managers across the agency, who meet quarterly to ensure high quality service delivery and outcomes.
SOME offers walk-in hours for Case Management services each weekday.

Together4Health
The Together4Health program uses a small team approach to help individuals living with chronic health challenges to get the care they need and to reduce the burden of managing their conditions, so they can focus on other goals and aspects of their lives they used to enjoy.
